Please complete the following to provide feedback on your childs Coach/Staff Member throughout the season.  This feedback and evaluation form can be completed anytime during the season should you wish to acknowledge the individual's performance or highlight any concern you feel needs to be addressed by an executive member of the WSA.  This information will remain confidential, and will also be used to help develop and determine our volunteers for the future.  All of your responses and comments will remain anonymous.  Regardless, all feedback and evaluations will be shared with the specific Coach or Staff Member.  Should you wish to be contacted by an Executive Member of the WSA, kindly indicate that at the bottom of the form, and be sure to include your name and contact info.  We appreciate you taking the time in completing this form.
